[Hand Expression 101]How to Hand Express

By Rachel Johnson, CNM, ARNP, CLC
1.How do I position my hands?
Place your thumb on top of the breast and your fingers underneath in a “C” or “U” shape, about 1–1.5 inches behind the nipple.
2.What motion should I use?
Use a press-back → compress → release rhythm. Avoid pulling or sliding your fingers along the skin.
3.How long does a session take?
Anywhere from 5–20 minutes, depending on your goal and how quickly your milk flows. Before birth and in the first few days after birth, 5-10 minutes a session is great.
4.Should I warm up first?
Warmth and gentle massage can help stimulate letdown and make expression easier.
5.Do I switch sides?
Yes. Alternate breasts or stay on one side until the flow slows, then switch until you have reach your goal.
